By accessing or using the Kaizen Horizons website, services, or programs, you agree to the following terms:
1. Services Provided
Kaizen Horizons offers holistic personal training, massage therapy, coaching, and related wellness services. Services may include virtual or in-person sessions, digital resources, and community access.
2. No Medical Advice
Our services are for wellness, education, and personal development purposes only. They are not a substitute for medical diagnosis, treatment, or professional healthcare. Always consult your physician before beginning any exercise, nutrition, or wellness program.
3. Client Responsibilities
You agree to:
Provide accurate and complete health and personal information before starting services.
Update us promptly if your health status changes.
Follow all safety instructions and guidelines during sessions.
Exercise caution and discontinue any movement or technique that causes pain, discomfort, or distress, and inform your trainer or therapist immediately.
4. Assumption of Risk & Waiver
You understand that physical activity, massage therapy, and bodywork carry inherent risks, including but not limited to injury, illness, and physical strain.
By participating, you voluntarily assume all such risks and release Kaizen Horizons, its owner, and any representatives from liability for injury, illness, damages, or loss arising from participation, except where prohibited by law.
5. Session Policies & Scheduling
Scheduling: Virtual and in-person sessions are by appointment only and may be booked via our secure online portal.
Cancellations/Rescheduling: At least 24 hours’ notice is required to cancel or reschedule in person and virtual sessions without charge or forfeit.
Late Arrivals: Sessions will end at the scheduled time, regardless of late arrival.
Trainer/Therapist Availability: The practitioner is available for 24 hour response time to questions, concerns, and form review Tue-Sat. And is unavailable on designated days off (currently Sundays and Mondays) and will respond to client communications within a reasonable timeframe on working days.
6. Payment and Refunds
Payment is due at time of service.
Subscription and package fees are billed automatically according to the agreed plan.
Refunds are assessed on a case by case basis once services have commenced, except in documented extenuating circumstances (medical emergency, illness, or injury preventing participation), in which case the owner will evaluate and up to 100% of the unused portion of service time may be refunded with appropriate documentation from a licensed provider.
Prices are subject to change; notice will be provided for ongoing subscriptions.
7. Intellectual Property
All training materials, videos, written guides, and program content are the property of Kaizen Horizons. You may use them only for your personal use related to the services provided. Reproduction, distribution, or sharing without written permission is prohibited.
8. Privacy and Confidentiality
We protect your personal and health information in accordance with our Privacy Policy and HIPAA Notice of Privacy Practices. Personal and health information is kept confidential except as required by law or with your written consent.
9. Technology & Communication Disclaimer
While we use secure platforms for client communication, you acknowledge that electronic communications (email, text, video conferencing) carry some security risks. By choosing to use these methods, you accept those risks.
10. Termination of Services
We reserve the right to refuse or terminate services without refund if necessary for safety or policy violations.
11. Dispute Resolution & Governing Law
This agreement is governed by the laws of Washington State. Disputes will be resolved through mediation or arbitration before legal action.

12. Entire Agreement & Modifications
These Terms, together with any signed service agreements and our Privacy Policy, represent the entire agreement between you and Kaizen Horizons.
